Hospice Eligibility

Many healthcare providers have found it difficult to pinpoint an exact prognosis and have questions about the right time for hospice intervention. Below, you will find guidelines for hospice eligibility among the most common hospice diagnosis. If your patient meets some or all of these criteria, it is time to consult Community Hospice for an evaluation.

Rapid decline in your patient’s condition, comorbidities, or physician’s clinical judgment may also support hospice eligibility.
